RB&C Adoption Health Service Post-Adoption Family Program


Overview

Adoption Partners: The purpose of the program is to decrease parental stress and enhance parent-child relationships in adoptive families. The program targets adopted children who are at-risk of health, developmental and socio-emotional difficulties as a result of pre-adoptive experiences. It is designed to strengthen and preserve adoptive families, improving family functioning and successes in adoption.  Adoption PARTners (AP) refers to the major activities implemented through this project: Parent-Centered, Assessment, Referral and Transition. It is an evidence-based model of service and will continue to collect information and monitor the effectiveness of the services offered.

Services offered
  • Assessment. Before any intervention begins, we will conduct developmental and behavioral assessment of the child and a parent/family assessment. The results of all assessments will be shared with families.
  • Intervention. The intervention is for a maximum 6 month module. Families can become involved for 3 months and at the end of 3 months, renew their participation in the program. Since we do not want families to become dependent on services, it would be unusual for a family to continue with the program after 6 months.
    • The intervention is home-based if the family lives within 30 minutes of the clinic. If not, the intervention will be clinic-based.
    • The intervention will involve one or both parents; the family can choose.
    • The intervention is strength-based. It models techniques that the family can use to improve the child's socio-emotional development, attachment, and behavior management, helping families see what they do well with their children.
    • The intervention incorporates family systems. It provides social support to the family and examines issues in the family from an ecological perspective. As such, case management is part of the intervention.
  • Referral. As part of monitoring our efforts with each case, we will make referrals to other services-medical, early intervention, parent counseling-as needed and appropriate.
  • Monitoring the intervention. The interventionist has regular meetings with a team to talk about cases, the progress of cases, and any issues emerging in cases. We repeat the assessments conducted at the beginning at 3 and 6 months to monitor progress, again giving feedback to the family. 
  • Termination & Transition. After services are terminated, families are invited to participate in a monthly parent support group meeting in their community.
Costs

            The cost for services is $800 per month for the formal services that last up to 6 months. Families are encouraged to apply for PASSS funding to help cover the costs for the program
